Since there's no way to appeal for forum accounts ban, you have to appeal normally.

Steps:
Log in with your character through UCP, click on "Server" sign and then Ban Appeals ( http://sa-mp.im/server/ban-appeal ).

How to fill the format:
Put the name of the admin who banned you, or state unknown if you don't know him.
State that its a forum ban,ban reason and your forum name (However, its not needed in the title, can be just stated in the appeal).
Example:
Yes, I know that its not such an important guide for the most of you, however I've seen questions about it.

Temporary forum bans are never lifted if they are shorter than 14 days. Sit them out.
If you have been IP or permanently banned from the forum, send a private message to woot.

What if someone has been unfairly banned for a while? People just use it and need it , this isn't the place for showing off your smartness .
