Go at this section : viewforum.php?f=22
Press the "New Thread" , make sure you use the correct title "Firstname Lastname [Issue]" for example "Toby_Wilks[Password Crashed]
Then you start following the format and the rules too , you should try post as more evidences as you got :

[b]Ingame name:[/b]
[b]IP:[/b]
[b]Issue:[/b]
[b]Evidence[/b]

for example :
Ingame name: Toby_Wilks
IP: Your IP.
Issue: You write what happened etc.
Evidence You post here some screenshots ( you can find them at : My Documents/GTA San Andreas User Files/screenshots ) and you post your phone number in-game , level , how much money approximately you had , if you had houses , vehicles etc.
