Constant Desyncing

I desync randomly, No mods, Original files.

Do you play other servers to comment on if this only happens on IM or other servers too?
Can you open the player list (tab), double click your name and post a screenshot of the network statistic box that is displayed.

23.3% Packetlost and it happens on others too

Well that's your issue.
Do you use wifi to play?
If so, how far away from the router are you?

Has this always been an issue for you or has it only just come about?
Have you tried simply restarting your router?
How about using an Ethernet cable to test if the connection becomes more reliable?
Also can you do the following while connected to your wifi and post a screenshot of the results here.

Press Win + R
Type cmd.exe
Type tracert www.sa-mp.im
If you can test using Ethernet, then do the same as above for that too and post a screenshot of the results.

What is your machine spec? And do you have any mods? Your packetloss of 11%-23% will be causing the desyncing, it suggests a bad connection/fault somewhere along the network. But you've also got an FPS of ~30?
